PH is a 63-year-old woman with a history of myocardial infarction who comes to your office at her family’s insistence for evaluation of shortness of breath. “My sister lives two blocks away, and I’ve walked over to her house without any problem for the past 10 years. Now I have to stop three or four times because I can’t breathe. And sometimes I wake up in the middle of the night gasping for air.” During the physical exam, you hear an S3 heart sound and detect 2+ lower extremity pitting edema. You review her office records and note that neither of these were present during previous appointments.
