An 80-year-old man reports that he occasionally “passes out.” He states that this only occurs when he tilts his head back. He has a 50-pack-year smoking history, well-controlled diabetes, and hypertension. He denies alcohol or drug use. He has had a left carotid endarterectomy as well as surgical repair of an abdominal aortic aneurysm several years ago. The physician informs him that as people age, their risk for atherosclerosis increases. The physician tells the patient that he is likely experiencing transient brainstem ischemia when he extends his neck.
